Bilingual Housing Navigator
Cascade AIDS Project(CAP) is the oldest and largest AIDS Service Organization (ASO) in Oregon and Southwest Washington that provides the LGBTQ community and beyond with compassionate healthcare. CAPs mission is to promote wellbeing and advance equity by providing inclusive health and wellness services for LGBTQ people people affected by HIV and all those seeking compassionate care. This is a FullTime role working at our Davis St. location in the Old Town area of Portland OR but will travel frequently throughout the fivecounty service area for meetings and client home visits. Evening and weekend work is occasionally required. The schedule for this role is 8:30am5:00pm
Starting pay for this position is $48452.19 a year. The Bilingual Housing Navigators Pay increases with tenure and the top of the pay scale is $56252.19. This is a union represented position so the compensation benefits and conditions of work are collectively bargained.
Who You Are
- Verbal and written fluency in English and Spanish
- Bachelors degree in human/social services field (social work public or community health psychology) or related field and/or two years of experience relevant to the position
- Prior experience providing 12 years of successful case management (or similar) services
- Demonstrated computer proficiency using Microsoft Office software (Word Excel Outlook) and working knowledge of the internet
- Excellent written and oral communication skills.
- Successful experience working with ethnic racial economic and sexually diverse populations and persons who have experienced homelessness persons with a mental illness and/or substance addiction
What Youll Do
- Carry an active client panel of approximately 30 program eligible clients assisting with eviction prevention housing planning advocacy mediation and information and referral.
- Provide bilingual supportive case management housing navigation housing placement evictionprevention support and information and referral with and on behalf of clients who are homeless or at risk of becoming homeless using a supportive strengthsbased model that promotes client selfdetermination and independence.
- Process and place CAP motel vouchers as a part of the Motel Team.
- Complete CAP Intakes and conduct comprehensive assessments (including MSST assessments for and goal planning to understand housing needs and potential barriers to housing for clients. Develop individualized service plans based on these assessments and available resources.
- Complete Ryan White Eligibility Determinations.
- Assist clients with housing applications complete Coordinated Access housing assessments (MSST) complete supportive and subsidized housing paperwork survey rental market for affordable housing and advocate for clients with prospective landlords
