ESSENTIAL FUNCTIONS AND BASIC DUTIES:

- Meets the patients goals and needs and provides quality care by assessing and interpreting evaluations and test results; determining physical therapy treatment plans in consultation with interdisciplinary team.
- Diagnoses and manages movement dysfunction and enhances physical and functional abilities.
- Restores maintains and promotes not only optimal physical function but optimal wellness and fitness and optimal quality of life as it relates to movement and health.
- Prevents the onset symptoms and progressions of impairments functional limitations and disabilities that may result from diseases disorders conditions or injuries.
- Recommends use of assistive devices and provides gait training as deemed necessary.
- Completes discharge planning by consulting with physicians nurses social workers and other health care workers; contributing to patient care conferences.
- Documents patient care services via EMR charting using defensible documentation in a timely and concise manner in compliance with industry standards.
- Maintains safe and clean working environment by complying with procedures rules and regulations.
- Protects patients and employees by adhering to infectioncontrol policies and protocols.
- Maintains professional and technical knowledge by attending educational workshops; reviews professional publications; establishes personal networks; participates in professional societies.
- Develops rehab and hospital staff by providing information; developing and conducting inservice training programs.
- Complies with federal state and local legal and certification requirements by studying existing and new legislation; anticipating future legislation; enforcing adherence to requirements; advising management on needed actions.
- Strives to promote health creates a safe and effective treatment environment motivates others has a good bedside manner listens analyzes information and is quality focused.
- Participates in ongoing professional development to maintain a level of clinical practice consistent with best practice standards. This includes seeking opportunities to gain new knowledge and skills and utilizing these newly acquired skills in the provision of care and for growth and development of the department and hospital.

Schoolbased physical therapists (in addition to above expectations):
- Collaborates with the schoolbased team to help determine and carryout Individualized Education Programs (IEPs); identify a students annual goals and determine the services supports modifications and accommodations that are required for the student to achieve them.
Home health physical therapists (in addition to above expectations):
- Provide inhome physical therapy services to homebound patients and works in collaboration with the home health interdisciplinary team.

- Actively participates in departmental and organizational performance improvement and continuous quality improvement activities.
- Strives to uphold regulatory requirements to ensure continual compliance with departmental hospital state and federal regulations and policies.
- Follows policies and procedures for infection control safety and risk management to ensure a safe environment for patients the public and staff.
Salary range: $83158.40 $124737.60
