Physician Adult Primary Care

FullTime or PartTime
Exact Schedule Negotiable
No Evenings
No Weekends
No Holidays
FULLTIME COMPENSATION:
$180000 to $225000 Base Salary depending on experience

COMPANY PROFILE:
This organization is dedicated to delivering convenient highquality primary care to geriatric patients in the comfort of wherever they call home. They currently boast a team of more than 20 physicians and nurse practitioners who ensure that patients receive the best medical care possible without the challenges of traveling to a providers office. Their services include among others primary care chronic care psychiatric care postacute care transitional care and remote patient monitoring.
POSITION DESCRIPTION:
The physician will serve as the primary care provider for a dedicated panel of geriatric patients.
All patients are located in facilities or private homes. Depending on the territory the practice is able to offer different mixes of facilities (ALFs / ILFs / SNFs) and homebased patients. During the initial phone interview the practice will look at the physicians home location and discuss this in detail mutually agreeing upon a geographic territory and patient settings.
In SNF settings (with no travel during the day only one location per day) physicians see an average of 15 patients per day.
If traveling between ALFs ILFs and homes physicians see an average of 9 to 12 patients per day.
Specific duties of the role include but are not limited to:
conduct health assessments to identify patient needs
evaluate patient conditions and adjust treatment plans as necessary
perform minor procedures and wound care as required (and as settings permit)
educate patients and families on health management disease prevention and treatments
work with other providers as needed to coordinate comprehensive care
The practice uses the eClinicalWorks EHR.
REQUIREMENTS:
Ohio medical license
DEA
BC/BE status
no experience required
