Your Role
As an Occupational Therapist you will support individuals to maintain their independence in the community through the development maintenance or accommodation of functional daily living skills. It is responsible for assessing the developmental level of functioning the development of a therapy plan reviewing needs and assessing for adaptive equipment as well as providing ongoing occupationaltherapy services. This position also participates in psychoeducational programs.


Responsibilities
  • Develop an assessment to evaluate the impact of a persons mental physical or medical impairments on maintaining independence in a community setting.
  • Complete evaluation of consumer to establish a personalized therapy plan including but not limited to reviewing medical history asking questions and observing them doing tasks to achieve the most effective results.
  • Evaluate a consumers home or workplace on the basis of the consumers health needs to maintain independence in a community setting.
  • Administer therapy approaches and instruct consumers with therapy activities and tasks that promote rehabilitation of consumers injuries and traumas.
  • Provide direct services that support individual in the deve.lopment of homemaking skills activities of daily living independent transport within community recreational activities and social activities through occupational therapy intervention and approaches within a recovery model.
  • Educate a consumers support system or caregiver(s) about how to accommodate and care for the consumer.
Qualifications
  • Bachelors Degree and current license in Occupational Therapy.
  • Minimum of one year experience working with individuals with mental illness and working knowledge of the recoverymodel.
  • One year case management linkage outreach and/or community support experience.
  • Experience with Medicaid Rule 132 documentation and billing requirements.
  • Valid Illinois drivers license and documentation of current auto insurance with good driving record and car available.
  • Proficient in the use of computers software applications and working knowledge of Microsoft Office Suite programs.
