Park Nicollet is looking to hire a Licensed Independent Social Worker (LICSW) to join our Inpatient Hospice team! Come join us as a Partner for Good and help us make an impact on the care and experience that our patients and their families receive every day.

Position Summary:

TheLicensed Independent Social Worker (LICSW) provides direct social work services to patients extending services to family members in need of identified social worker services providing them with psychosocial and environmental support.

Makes available a full range of treatment modalities with individuals families and groups based on social work assessment and diagnosis.

Coordinates social work services with other departments and providers within the Park Nicollet Health Services i.e. hospice hospital and clinic based social workers and out of system providers.

Provides social work services to the patient and associated community.

May provide clinical supervision to other Social Workers.

Works with physicians nurse clinicians other medical/surgical specialists and other Park Nicollet Health Services employees.

Contact with patients and their families.

Required Qualifications:

  • Education Experience or Equivalent Combination:
    • Masters degree in social work
  • Licensure/ Registration/ Certification:
    • Must be licensed as Licensed Independent Clinical Social Worker (LICSW) or LISW.
    • Current license to practice social work in the state of Minnesota
  • Knowledge Skills and Abilities:
    • Requires basic computer/keyboarding skills.
    • Demonstrates ability to conduct individual and group therapy sessions.
    • Demonstrates ability to coordinate and facilitate group interactions.
    • Demonstrates knowledge of agerelated growth and development principles necessary to provide appropriate service and assure safety of patients. Individuals are responsible for the travel arrangements required to meet the expectations of the position

Preferred Qualifications:

  • Knowledge of word processing spreadsheet and presentation software desired
