Requirements:
Unrestricted State of Connecticut medical license (or eligible)
BC/BE Internal or Family Medicine or Geriatrician
Physician Per Diem SNF Admission Coverage
Position Details:
Hartford HealthCare is seeking a per diem Internist Family Medicine or Geriatrician on a per diem basis to provide Skilled Nursing Facility (SNF) admissions and rounding coverage.
Role: Geriatrician
Location(s): SNFs in the Bloomfield area
Setting: 100 Skilled Nursing Facility (SNF) patient admissions and rounding
Schedule: Per Diem as needed (seeking 45 hours/day 34 days a week)
Call: No call

- Supported by team of APPs that are in the facilities full time within a team of 18 Geriatric providers
- Strong support staff nursing and operational team plus strong peer support to assist with any challenging situations that may arise
