Job Description

Overview

The Outpatient Pharmacist provides pharmaceutical care supervises support staff and participates in dispensing activities adverse drug reaction monitoring drug information and patient education. This person may provide pharmacy services to patients from neonatal infant child adolescent adult and geriatric age groups. This person will have knowledge of the skills to care for the pharmacy needs of these populations. Pharmacists will have oversight responsibility when nonlicensed staff handle noncontrolled substances.

Responsibilities

Patient Care
* Performs clinical services to optimize drug therapy and documents interventions.

* Delivers medications to the patients bedside prior to discharge and provides medication counseling.

* Ability to prepare specialty compound .

* Provides accurate clinically relevant drug information to physicians nurses and allied health professionals.

* Provides pharmaceutical care to nursing facility patients.

* Identifies prevents and investigates potential and real adverse drug reactions and medication errors.

* Provides medication counseling to patients at the pharmacy level.

* Reviews and interprets physician orders properly; screens for incompatibilities interactions duplicates or contraindications.

* Contacts prescriber and/or Nursing personnel regarding any questions on drug orders.

* Responsible for the accurate dispensing and labeling of medication orders.

* Competent using various pharmacy technology products such as electronic health records automated dispensing machines unit dosing and clinical decision support/data mining tools.

* Provides drug information to prescribers per request.


Administrative
* Processes patient charges and credits.

* Assists in maintaining basic drug supply from vendors.

* Exercises applicable legal and professional controls on controlled substances.

* Verifies physical inventory for incoming/outgoing items.

* Supervises supportive personnel and organizes/directs workflow as needed.

* Meets administrative and miscellaneous requirements of the department.

* Reports adverse drug reactions medication errors and good catches.

* Double counts the money on the point of sale system at the end of the day.

* Performs all operational aspects of retail pharmacy including computer operations and electronic billing.

* Serves as a resource to staff in caring for complex patients.

* Works with the pharmacy manager and other pharmacy team members to achieve compliance of department policies and procedures.

* Assists the department manager and other pharmacy staff on all aspects of quality improvement of pharmacy services.


Compliance/Safety
* Responsible for reporting any safety related incident in a timely fashion through the Midas/RDE tool; attends all safety related training programs; performs work in a safe manner; monitors work environment for possible safety issues and ensures others are also performing work in a safe manner.

* Adheres to regulatory and organizational standards affecting patient care and safety as well as all hospital and departmental policies and procedures. Practices in a safe and cost effective manner.

* Stays current and complies with state and federal regulations/statutes and company policies that impact the employees area of responsibility.

* Ensures all certifications and/or licenses are uptodate and valid prior to expiration dates.

* Completes all company mandatory modules and required job specific training in the specified time frame.

Qualifications

Education
Bachelors Degree in pharmacy from an accredited college of pharmacy Required


Certification & Licensures
Arizona State Board of Pharmacy license Required


Experience
One 1 year of experience in a retail or alternate care pharmacy Preferred

Healthcare is a rapidly changing environment and technology is integrated into almost all aspects of patient care. Computers and other electronic devices are utilized across the organization and throughout each department. Colleagues must have an understanding of computers and competence in using computers and basic software programs.
