Job Description
ProMedica Physicians Group (PPG) is seeking a dedicated and compassionate Advanced Practice Provider to join our healthcare team at ProMedica Flower Hospital in Sylvania OH.
HIGHLIGHTS:
- Schedule: 5 nights on 10 nights off.
- Weekend/ Call Requirements: Rotating coverage once a month
- Current team consists of 7 providers
- Support staff consists of 8 MDs 28 APPs 1 lead APP and a Practice Director
- Deliver comprehensive medical care to patients 18 years of age including conducting physical examinations diagnosing and treating acute and chronic conditions and prescribing medication to patients 18 years of age
- Develop individualized treatment plans based on patient history and medical assessments while collaborating with a multidisciplinary team to ensure highquality coordinated care.
- Play a key role in educating patients on preventive care healthy lifestyle choices and disease management contributing to the overall wellbeing of the community. Essential for 360 base care that Levis IM providers the community.

Requirements
- Certificate of Authority to practice.
- Current American Heart Association (AHA) CPR certification.
Physician Assistant:
- Mastersdegree
- Must be licensed or license eligible in the state of Ohio
- Certificate of Authority to practice is required.
Nurse Practitioner:
- Master of Nursing (MSN) or Doctor of Nursing (DNP) degree
- Must be licensed or license eligible in the stateof Ohio.
