Job Description
We are searching for a dedicated MTM (Medication Therapy Management) Pharmacist to join our dynamic healthcare team. In this role you will play a crucial part in optimizing medication therapy for patients conducting comprehensive medication reviews and collaborating with healthcare providers to ensure the highest quality of patient care. The ideal candidate will possess a background in medication therapy management or a willingness to learn. Need to have a commitment to patient wellbeing and excellent communication skills.
Preferred Qualifications:
- PharmD degree from an accredited pharmacy program
- Experience in Medication Therapy Management (MTM)
- State pharmacy license in good standing
- Proficiency in utilizing MTM platforms and tools
- Strong analytical and problemsolving skills
- Excellent communication and collaboration abilities
- Certification in MTM services
- Familiarity with electronic health record (EHR) systems
- Dedication to ongoing professional development and continuous improvement
