Location: Horsham
Rate: 38 per hour Ltd. or 35 per hour with 150 weekly expenses (for those living outside of West Sussex)
Liquid Personnel is seeking a dedicated qualified Social Worker to join our clients Mental Health Hospital Discharge Hub based in Horsham.
What will your responsibilities be
In this role you will complete all statutory work such as Care Act assessments s117 aftercare planning meetings safeguarding Mental Capacity Assessments and Best Interest Meetings. You will work proactively under pressure to ensure all aspects of the individuals needs are considered and incorporated into the discharge process. You will support customers carers and families sometimes outside of normal working hours. Additionally you will travel independently throughout the county including areas not easily accessible by public transport making a driving licence essential.

Qualifications and Experiences:
To be successful in this role you must have a degree in a relevant professional qualification (e.g. Certificate of Qualification in Social Work or Diploma in Social Work). You must be registered with the relevant professional registration body and maintain that registration. Additionally you should have an Approved Mental Health Practitioner (AMHP) qualification or be working towards it to be completed within 2 years of appointment. A full valid driving licence and double vaccination against COVID19 are also required.
