Job Summary
The Housing Liaison is a specialty case management position in addition to the roles and responsibilities of a case manager the Housing Liaison is responsible for supporting individuals with the process of eviction prevention provided through Coronavirus Local Fiscal Recovery Fund pursuant to the American Rescue Plan Act (ARPA) in collaboration with the City of Phoenix and Mercy Care. The Housing Liaison will work in the community to establish relationships with internal and external agencies for the referral process in accordance with identifying individuals that meet the criteria for the eviction prevention program. The Housing Liaison will be responsible maintaining and processing all referrals working within the Bellweather system coordinating with the Finance Department and delivering eviction prevention funds to individuals. The Housing Liaison works within a multidisciplinary team and provides supports to all individuals that are facing potential evection if support is not provided.
Skills/Requirements:
- HighschoolDiploma or GED required. Associates Degree or higher in a field related to behavioral health preferred.
- Minimum of six 6 months experience in the Behavioral Health and/or Substance Abuse field required.
- Lived experience in recovery from mental health and/or substance use disorders homelessness family members and veterans preferred.
- Current AZ Drivers License in good standing.
- Clear 39month Motor Vehicle Record.
- Behavioral Health Technician (BHT) in accordance with A.A.C. R910101.33 and CBI Clinical Policies and Procedures.
- Certified Peer Support Specialist (CPSS) preferred.
