Job Description
Granite VNA is hiring Home Health and Hospice LNAs! This is a per diem flexible job opportunity.

The Home Health and Hospice LNA helps achieve our mission by performing personal care and designated healthrelated services for Hospice patients in their homes and/or in a facility. The Hospice Community LNA interacts with clients family members facility staff and other healthcare team members. Some essential functions include:
- Follows care plan in providing basic care and services to the resident including assistance with Activities of Daily Living (ADL): walking bathing shaving mouth care grooming eating transfers and management of incontinence
- Communicates with nurses involved in the residents care plan; observes records and reports changes in residents condition status or other pertinent information to the nursing staff
- Laundry housekeeping and maintenance to assure a safe orderly clean and comfortable environment
- Provides supervision to patient/residents with cognitive deficit
- Ensures clients have the opportunity to engage in cultural spiritual physical political social and intelligent activities as the residents desire
- High school diploma/GED accompanied by current New Hampshire LNA license
- Reliable automobile and auto insurance
- Valid Drivers license
- If hired be available MondayFriday for the first weeks of employment to complete the required training.
