Job Description
Job Title:Social Worker (Hospital Discharge Team)
Location:Middlesbrough Metropolitan of Lewisham
Rate:284 per day
An exciting job role has recently become available at the Middlesbrough Metropolitan of Lewisham. They are looking for a new member for their Hospital Discharge Team.
X4 workers required for a 12 month period
2 years postqualifying experience required. Request to cover case work/reviews. This will involve the ability to undertake facetoface visits 5 days a week with 2 days a week working from an office setting. Learning Disability Team adults in transition and over 18 with a learning disability.

Requirements:
- Assessing Needs:Conduct interviews and assessments to understand the needs of individuals and families.
- Developing Care Plans:Create and implement care plans to support clients in achieving better outcomes.
- Advocacy:Advocate for clients rights and access to necessary services.
- Collaboration:Work with other professionals in multidisciplinary teams to provide comprehensive support.
What we are looking for in a Social Worker:
- Social Work England registration
- Eligible to work in the UK
- Hold a full UK licence
- Postqualified experience
- Degree level or equivalent in Social Work
