About the AEIT Team
This role is based in the Adult Early Intervention Team (AEIT) which is part of the Hospital and Health Partnerships Service.The team is made up of a team manager lead practitioners assessment and enablement officers as well as occupational therapists. The team supports residents postdischarge reviewing their care and support needs to ensure they receive the right care to enable them to live as independently as possible in the community. This role includes reviewing reablement provision whether initiated by the hospital social care teams Social Care Direct or the MultiAgency Safeguarding Hub and assessing any long term care and support needs.

The team takes a personcentred strength based approach ensuring individuals are involved in their care and support planning advocating for the right support from health and care partners. Partnership working within the local authority and with external stakeholders is key to delivering effective and wellcoordinated care.

About the role
  • Support the Team Manager to provide clear and responsive practice leadership and supervision and promote the highest professional standards in social care practice.
  • Promote a culture of continuous learning and professional development of staff and students across Communities Adults and Health to enhance the quality of practice and staff wellbeing.
  • To support people to maintain their health wellbeing and independence by providing access to good quality information and advice and signposting to local preventative services with a focus on building their resilience and strengths to stay well and achieve the outcomes that are important to them.
  • To provide a high standard of social work to adults and/or carers arising out of the Councils duties and powers under the Care Act 2014 and in accordance with Barnet policies and procedures guidelines and the Code of Practice for social care workers.
  • To provide appropriate professional social work assessments with adults and carers with a focus on meeting the social care outcomes as articulated in the Department of Healths Adult Social Care Outcomes Framework.
  • To safeguard adults and carers at risk of harm and promote Making Safeguarding Personal principles and their wellbeing.
  • To undertake and review individualised and outcomesled care and support. To ensure that the wellbeing principles are at the heart of assessment and care and support planning.
The successful candidate will be required to carry a reasonable caseload and complete facetoface assessments. Working hours are Monday Friday with the opportunity for remote working. Support will be provided to upskill the successful candidates in terms of providing supervision of staff and managing the team.

Fully competent to Senior Social Worker level under the Social Worker National Competency Framework

Has relevant social work qualification and registered with Social Work England

Work collaboratively with partners maximizing joint working leading in agreed areas of work for both Social Care and the integrated care board.

Provide professional supervision and mentoring to employees supporting caseload management. Promote reflective practice and continuous professional development.

Participate in the completion of the decision support tool applying sound judgment based on the evidence obtained. Complete and contribute to care act assessments reviews and reassessments working within the legal context local policies and procedures.

Promote strengthbased working and identify training needs.

Agree to work as appropriate and under the guidance of senior management to maintain budgetary recommendations to ensure the best value for money.

Work alongside external organisations and manage conflict as necessary.

Have a good understanding of the appeals and dispute policy representing the local authority client and families during the appeal and dispute process.

Manage competing pressures within the service area and ensure that the work is allocated and delegated as appropriate.

Be Selfmotivated and have excellent organisational verbal and written communication skills

Contribute to the development of the service being fit for the future by working within the legal framework local policies and agreed procedures.

Maintain robust records of cases within the service area and provide reports when required.

Have the desired experience skills and expertise to represent clients families and the local authority in complex challenging situations.
