Job Description
An exciting job role has recently become available at Peterborough. They are looking for a new member for their Transfer of Care Team.

Responsibilities:
- Assess individuals in hospitals for care packages or care home placements under the Care Act.
- Conduct Mental Capacity Assessments for care and support.
- Collaborate with multidisciplinary teams to create care plans.
- Engage in carers conversations.
What we are looking for in a Social Worker:
- Social Work England registration
- Eligible to work in the UK
- Hold a full UK licence
- Postqualified experience
- Degree level or equivalent in Social Work
