NYAP is participating agency in the Great Minds Fellowship and Great Minds Workforce Commitment Incentive Programs.
Stabilization Therapists with National Youth Advocate Program work in the community providing services that make meaningful impacts for youth and their families. This position works with high acuity/ needs youth with the focus of stabilization. Under the general supervision of the Clinical Supervisor or designee this position works closely with youth family foster parents and community partners providing advocacy and support toward the ultimate goal of living safely and successfully in the community.
Responsibilities
Provides appropriate individual group and family mental health services intervention and other evidence based clinical services as required.
Travels daily to provide community based services to and on behalf of youth and families in compliance with organizational contract and regulatory requirements.
Coordinates and monitors clinical services for the youth and family in the community including mental health special educational services medication management and psychological services.
Regularly reviews and modifies youthbased plans.
Establishes and maintains strong relationship with foster parents and families of origin.
Assists in family reunification and stabilization efforts with the youths primary family or other permanent living arrangement consistent with the youths case plan.
Provides safety assessment crisis response and behavioral stabilization services as required.
Flexible availability including weekends and scheduled oncall rotation.
Minimum Qualifications
Masters Degree from an accredited institution.
A valid professional license (LSW LPC) as required and acceptable by state or contract regulations.
Experience working with youth and foster care required.
Must have flexible availability including evenings.
Proficient use of desktop and laptop computers smart phones and tablets printers fax machines and photocopiers as well as software including word processing spreadsheet and database programs.
