The Lead is an integral member of the Patient Reported Measurement program within PersonCentred Care and plays an essential role leading the development planning implementation of projects and operational initiatives that support clinical quality improvement innovation and research leveraging best practices and evidence within the Population Health and Value Based Systems Portfolio. This role will collaborate closely with internal program and health system leaders as well as external stakeholders (e.g. Ministry of Health Ontario Health Teams) to prepare presentations/reports/briefing notes/etc. organize and lead meetings manage project timelines as well as other assigned activities. This role will manage ongoing wellestablished and new areas of work focused on patient reported experience and outcome measures and work closely across the clinical programs with clinical leaders and various stakeholders to drive clinical service changes broader health system improvements and health system performance reporting.
Here is what you will be doing:
Plan lead manage and execute projects of high priority and complexity with provincial scope related to the Patient Reported Measurement program.
Work closely with site leads clinicians and other stakeholders to identify opportunities to align clinical quality improvement and operational priorities across different health care sectors and clinical program areas (such as Ontarios Hip and Knee PROM Program).
Develop and maintain internal OH relationships and regional team relationships to advance regional clinical programs and innovation initiatives.
Utilizes data and evidence to help identify areas for clinical quality improvement. Additionally coaches and supports programs to effectively use patient reported data with the goal of advancing person centred care.
Actively manage projects/initiatives including identifying and effectively communicating with relevant stakeholders managing project documentation driving agendas and providing input in a collaborative setting.
Manage multiple projects and operational tasks concurrently.
Manage and coordinate engagement activities with provincial and regional stakeholders as well as other stakeholders through planned engagement sessions including working groups expert committees advisory councils and focus groups.
Proactively identify and manage project risks and issues and manage escalation processes as required.
Support system and regional level performance measurement and management with an emphasis on using data for improvement and planning.
Act as a key interface and liaison with Managers Directors and Executives as well as other key internal and external stakeholders including across Ontario Health Canadian Institute for Health Information and Ministry of Health.
Advise and update management teams on a regular basis including challenges and opportunities.
Pull diverse and interdisciplinary groups together emphasizing interdependence shared overall objectives and the need for combined skills.
Work closely with Ontario Health Teams and Ontario Health clinical programs (i.e. cancer cardiac renal mental health etc. and support alignment and implementation of provincial regional digital and clinical standards and guidelines as appropriate.
Work collaboratively with other portfolios at Ontario Health to drive performance measurement and health system planning activities.
Support the provincial/regional advisory bodies and subject matter experts who provide guidance and thought leadership (i.e. innovating thinking) for the program.
Provide input into the use of data analytics and reporting to improve quality at all levels of the system using provincial and regional data holdings.
Provide leadership in publication research evaluation and the building of evidence to support the role and scope of patient reported measurement.
Develop content expertise in the data business intelligence and analytics that supports the Patient Reported Measurement program.
Work closely with Digital Excellence in Health teams to gather requirements and design develop and implement technical solutions for new and/or operational provincial/regional data collection and management digital platforms.
Support stakeholders in troubleshooting digital platforms data and business intelligence issues.
Support initiatives to expand Patient Reported Measurement program strategies at the population health level.
Monitor collate and synthesize information on evidence trends and best practices to facilitate planning and decision making.
Develop needs assessments jurisdictional scans options analyses impact assessments to create program policies and guidance.
Prepare reports briefings policy papers briefing/issue notes and presentations for a variety of audiences.
Develop and present material using excellent written verbal and graphical communications skills with ability to express complex concepts effectively to a variety of audiences.
Deliver excellent tactical and critical strategic thinking.
Approach problems proactively and recommend thorough and practical solutions to a wide range of complex problems.
Promote collaboration and teamwork including working for consensus and contributing to achievement of team/group goals.
Here is what you will need to be successful:
Education and Experience
An undergraduate degree (masters degree preferred) in public health health sciences business/health administration public policy or a related field is required
5 to 8 years of experience including at least two 2 years working within the healthcare sector and leading/enabling largescale strategic projects
Experience conducting endtoend planning and of program/operational activities within the specific focus area of expertise
Experience synthesizing and applying scientific research/evidence to quality improvement and health system design initiatives
Experience as a lead who is skilled in selfmanagement leading by example and setting objectives managing expectations and facilitating stakeholder engagement activities across politically sensitive environments
Experience in change management quality improvement process development and business/project management operations is preferred
Certification in change management is an asset
Knowledge and Skills
Knowledge of population health management and patient reported measurement is preferred
Knowledge of frameworks for evidence review and evaluation and policy development
Strong understanding of Ontarios health care system and continuum of care including clinical processes with ability to use clinical data for improvement
Proven ability to build enhance and maintain strategic relationships with clinicians local and provincial governments health service providers and other key stakeholders to work in a variety of team situations
Demonstrated ability to successfully execute on multiple largescale complex highrisk projects ideally within a crossfunctional structure
Proven ability to problem solve by providing thorough and practical solutions to a wide range of complex problems and approaching challenges proactively using strong critical thinking and negotiating skills
Proven ability to develop relationships with key stakeholders to establish trust credibility and respect and clearly articulate key messages that resonate across different stakeholder groups and audiences
Excellent writing skills including the ability to prepare briefing notes reports and business presentations
Excellent oral skills including ability to create and deliver effective presentations and answer questions
