Job Title: Licensed Practical Nurse LPN Visit Nurse
Compensation: $32 $33 per visit
Benefits: Full Benefits Package
Location: Twin Cities Metro Area
Schedule: FullTime or PartTime Availability

Position Summary: Home Care is in high demand and we are looking forexperienced and caring LPNs to join our team at Home Health Care Incorporated!Successful applicants will have a proven track record as an LPN with home careexperience a huge benefit but not required. Responsibilities:
- Provide patient care according to clients care plan and needs
- Perform wound care treatments and assist clients with medication management
- Teach the client and/or family members about care and support skills
- Communicate regularly with RN Case Managers Supervisors and the Care Team
Qualifications:
- Licensed as a Practical Nurse / LPN in the state of Minnesota
- Driving is required; must have a reliable vehicle with current insurance and a valid Drivers License
- Demonstrated ability to work autonomously
- Prior Nursing experience in a TCU Hospice or Home Health setting
- Experience with Electronic Medical Records (EMR) software is preferred

- Army: Practical Nursing Specialist (MOS 68C)
- Navy: Hospital Corpsman (HM)
- Air Force: Practical Nursing Specialist 4N0X1
- Marines: Hospital Corpsman (HM)
- Coast Guard: Health Services Technician (HS)
