Successful candidates may be eligible for a $10000 signing bonus and up to $5000 relocation assistance with a defined Return of Service contract.
Reporting to the Health Services Manager Primary Health Care Chronic Disease Management and Wellness the physiotherapist will utilize behavior change methods case management and client centered approaches in individual or group settings.The incumbent is team focused and will collaborate with Primary Health Care staff family physicians other health care providers and community groups. They provide guidance and support for clients including providing information about appropriate supports and programs that will meet health and wellness needs and social supports that can assist with daily living.The individual is responsible for exercise program development implementation and evaluation and must possess excellent leadership skills. The incumbent is a member of a highly collaborative interdisciplinary team and collaborates with colleagues to determine team goals and objectives participates in the evaluation of team effectiveness solves problems and shares responsibilities. The role includes completing intake assessments and facilitating group exercise programs. Excellent interpersonal and written communication skills are required. The incumbent will be required to travel to work in various community locations throughout the zone.
