Champion Rehab is an awardwinning rehabilitation program. As an Outpatient Clinic located on a larger healthcare campus our Therapists enjoy a variety of inpatient outpatient and inhome care options. As a Physical Therapist at Champion Rehab youll:

• Provide direct patient care: continuous assessment of patient needs and functional deficits patient care/treatment ongoing modifications to the existing plan of care as needed ability to initiate appropriate referrals to other disciplines when needed
• Work closely with a skilled and dedicated interdisciplinary team to maximize patient satisfaction and overall success based on diagnosis and comorbidities
• Apply clinical expertise as necessary including but not limited to family and staff education/training recommendations and guidance for safe and successful discharge supervision of Physical Therapy Assistant communicating with Physicians to ensure fluency of care
• Complete documentation which follows regulatory guidelines (state federal)

Qualifications:

• Graduate of an accredited Physical Therapy program
• Currently licensed to practice in the State of MN. New graduates and temporary license holders are encouraged to apply!
