Calling All Speech Language Pathologists! Make an Impact with Us!

Are you a dedicated Speech Language Pathologist (SLP) ready to make a difference in the lives of older adults Join our Rehab Team at Commonheart Home Health in partnership with Symbria and be part of a passionate employeeowned team where your contributions truly matter!

Position Highlights

  • Location: Cover the West Bend WI area with potential to travel up to a 30mile radius (we can consider a smaller territory).
  • Flexible Schedule: Set your own hoursno fixed start or end times!
  • No Home Health Experience Required!
  • Mileage Reimbursement according to company policy.
  • Clinical Fellows Welcome! We offer supervision and training.
  • We are open to:
    • PartTime: Regular weekly hours between 2029.
    • PRN Status: Maximum flexibility with no minimum weekly hours.

Responsibilities

  • Develop and implement personalized treatment plans to meet your patients needs.
  • Creatively execute clinical and educational programs to enhance speech therapy outcomes.
  • Collaborate with an interdisciplinary team while meeting productivity standards and completing required documentation.
  • Help patients achieve meaningful results and be a trusted partner to the communities you serve.

Qualifications

  • Graduate of an accredited school for Speech Language Pathologists.
  • Hold a valid state license.
  • Clinical Fellows are encouraged to apply!
  • While Home Health or related experience is a plus its not required.
