Job Description
Job Title: Social Worker Transition Team
Location: Bristol
Rate: 32 per hour
Bristol is actively seeking a fulltime Social Worker to join our dedicated Transitions Team.
What will your responsibilities be
In this role you will support our triage decisionmaking process produce care plans and work directly with individuals in Bristol who are transitioning to adulthood. You will ensure they receive the necessary support and guidance during this critical period.

Qualifications and Experiences:
To be successful in this role you must have Social Work England registration be eligible to work in the UK hold a full UK license have postqualified experience and hold a degree level or equivalent qualification in Social Work.
