Job Description
- Knowledge and experience in childwelfare family support practices social services and community service providers.
- Perform home visits to clients homes working closely with schools health providers social services community service providers etc.
- Additional duties include completing assessments facilitating team meetings writing plans of care documenting all activities referring clients to needed services etc.
Requirements
Education
- Minimum Associates Degree in Human Services or related field from an accredited college or university AND ample proven experience as a case manager required.
- Bachelors Degree in Human Services or related field from an accredited college or University and two 2 years related experience. (Foster Care Child Welfare Counseling etc. preferred.
- Associates Case Manager experience required.
- Bachelors 2 years related experience (Foster Care Child Welfare Counseling etc. preferred
- Must have a valid Drivers License and ability to maintain insurability throughout employment.

Two 2 years' related experience. (Foster Care, Child Welfare, Counseling, etc. Driver's License and ability to maintain insurability throughout employment required.
