Position Title: Aboriginal Health Practitioner (Parttime) x 1 Male & 1 Female

Location: Mandurah

About Us

At Nidjalla Waangan Mia (NWM) Aboriginal Health Service weve been dedicated to improving the health and wellbeing of Aboriginal and Torres Strait Islander communities in the Bindjareb region (Mandurah Pinjarra Waroona Yarloop and North Dandalup) since 2010. With a strong reputation for delivering culturally appropriate care to nearly 2000 active Aboriginal clients NWM continues to grow under management of Oseca a notforprofit communitybased organisation with a proud 30year history of empowering individuals and communities.

Position Overview

We are currently seeking two dedicated and passionate Aboriginal Health Practitioners to join our team on a parttime basis of 37 hours per fortnight being .5 of an FTE (flexibility is on offer).

In this role you will ensure accessible comprehensive care for Aboriginal communities by building strong relationships promoting culturally appropriate health programs and facilitating access to primary health services. You will support and empower individuals through health assessments home visits and care coordination and aim to strengthen and improve community health outcomes.

Key Components of the Role

  • Community Engagement: Build and maintain strong connections with local Aboriginal communities to support access to health services and address healthrelated concerns.
  • Health Promotion: Plan and deliver culturally appropriate health promotion programs aligned with the annual service delivery plan.
  • Client Support & Care: Conduct home visits health assessments and provide transportation while coordinating care with General Practitioners for Aboriginal health checks and followup.
  • Collaboration: Work with local health teams Aboriginal Liaison Officers and various health professionals to enhance access to services like chronic disease care health checks and PBS copayment measures.
  • Documentation & Team Participation: Maintain accurate records of client interactions and actively engage in team meetings and community consultations to ensure effective service delivery.

What you bring

  • Qualified Aboriginal Health Worker or Registered Nurse
  • Identify a person of Aboriginal or Torres Strait Islander Descent 50D)
  • Current registration with relevant professional body

Essential Requirements: To be considered for this role you must meet the following criteria:

  • The right to work in Australia
  • A valid WA drivers license
  • A valid and clear police check
  • A Working with Children Check (WWC)
