Job Description: LPN Skilled Nursing Facility

Are you a dedicated and compassionate Licensed Practical Nurse (LPN) looking to make a positive impact in a skilled nursing and rehabilitation facility

We are currently seeking LPNs to join our team at a skilled nursing and rehab facility in Pennsylvania to provide highquality care to our residents.

Position Details:

• Schedule: Monday through Friday 11:00 PM to 7:00 AM 7p7a or alternative shift

• Employment Type: FullTime

• Compensation: $25.00 $30.00/hour based on experience Shift Differentials

• Annual Salary:

  • At $25/hour: $52000 per year (based on 40 hours/week for 52 weeks)

  • At $30/hour: $62400 per year (based on 40 hours/week for 52 weeks)

Key Responsibilities:

• Deliver direct patient care while ensuring the highest standards of comfort and wellbeing for our residents.

• Assist with coordinating daily nursing operations including administering medications monitoring vital signs and providing wound care.

• Collaborate with the interdisciplinary team to implement and evaluate personalized care plans for residents.

• Ensure compliance with facility policies procedures and regulatory guidelines.

• Provide leadership and mentorship to nursing assistants and other team members to maintain a highfunctioning collaborative work environment.

• Communicate effectively with residents families and staff to address care needs and concerns.

Qualifications:

• Current and valid LPN license in Pennsylvania.

• Experience in a longterm care or rehabilitation setting preferred.

• Knowledge of longterm care regulations and best practices.

• Strong communication organizational and problemsolving skills.

• Compassionate and patientcentered approach to nursing care.
