Join a dedicated interdisciplinary team (IDT) focused on providing exceptional patientcentered care. The Physical Therapist plays a vital role in care planning and collaborates closely with the clinical team to support participants health and independence.
Key Responsibilities:
- Perform comprehensive physical therapy assessments in both facility and home settings to determine care needs preferences and goals.
- Actively contribute to IDT meetings developing personalized care plans for participants.
- Implement and document physical therapy interventions based on physician orders and care plans.
- Provide therapy services including active and passive exercises muscle reeducation gait training and ADL support.
- Administer treatments using techniques like massage heat sound water light or electricity.
- Supervise Therapy Assistants offering necessary training and guidance.
- Educate staff participants and caregivers on therapeutic equipment usage and care.
Requirements
Qualifications:
- Bachelor s Degree in Physical Therapy with 5 years of relevant experience.
- Master s or Doctorate in Physical Therapy preferred.
- Valid Physical Therapy License or proof of eligibility.
- 2 years of experience in a clinical setting ideally with elderly or frail populations.
- Foreign physical therapy degrees must be evaluated to ensure U.S. equivalency.
- CPR certification preferred.
