Job Title: Physical Therapist (PT) Rehabilitation & LongTerm Care
Location: Near Pittsburgh PA
Job Type: FullTime (Monday Friday)
Compensation: $45$52/hr
Job Description:We are seeking a motivated and skilled Physical Therapist (PT) to join our rehabilitation team in a longterm care facility near Pittsburgh PA. This position focuses on evaluating planning and providing therapeutic treatments to help residents improve mobility restore function and enhance their overall quality of life. The schedule is primarily Monday through Friday with occasional weekend coverage as needed.
Key Responsibilities:Conduct comprehensive physical therapy evaluations to assess residents mobility strength balance and functional abilities.
Develop and implement individualized treatment plans to promote recovery reduce pain and prevent further physical impairments.
Provide handson therapy therapeutic exercises and functional mobility training.
Collaborate with an interdisciplinary team including occupational therapists speech therapists nurses and physicians to ensure comprehensive care.
Educate residents families and caregivers on home exercise programs and strategies for continued progress.
Maintain accurate documentation of patient progress treatment plans and outcomes in compliance with facility and regulatory standards.
Participate in care planning meetings and staff training sessions as needed.
Ensure adherence to all state and federal regulations regarding physical therapy services.
Required: Licensed as a Physical Therapist (PT) in the state of Pennsylvania (or eligible for licensure).
Education: Doctorate (DPT) or Masters in Physical Therapy from an accredited program.
Experience in a skilled nursing facility (SNF) longterm care (LTC) or rehabilitation setting is preferred but not required.
Strong clinical reasoning and problemsolving skills.
Excellent interpersonal and communication skills to work effectively with residents families and staff.
Ability to work independently while also collaborating as part of a multidisciplinary team.
