Job Description

Job Title: Physical Therapist (PT) Outpatient

Location: Bad Axe MI

Shift Type: Days

Shift Times: 9:00 AM 5:30 PM

Schedule: Variable 72 hours per biweekly pay period

Hourly Pay Range: $37.78 $47.74 (based on a 5year wage scale; union position contract currently under negotiation)

Position Overview:

Join a supportive team as a Physical Therapist providing care to outpatient acute and swing bed patients. This role is focused on developing personalized patientcentered treatment plans that promote mobility reduce pain and prevent disability.

Key Responsibilities:

  • Evaluate patient conditions through detailed assessments systems reviews and specific testing
  • Create individualized care plans based on evaluation and physician orders
  • Provide evidencebased treatment to improve functional mobility and reduce pain
  • Continuously reassess patient progress and modify plans as needed
  • Begin and manage discharge planning starting from the first visit
  • Supervise and delegate tasks to ancillary staff and students as allowed by state law
  • Ensure accurate timely and legible documentation compliant with policies and payer requirements
  • Communicate effectively with patients families and healthcare team members using AIDET principles
  • Educate patients and families about conditions and care
  • Engage in ongoing professional development through training inservices and literature
  • Participate in quality improvement efforts
  • Maintain a safe environment for all patients and staff
  • Operate within the legal scope of physical therapy practice for the state

  • Licensed or eligible for licensure in the state of Michigan
