Job Description

SWAMS is a NotforProfit Aboriginal Community Controlled Health Organisation founded on the principles of selfdetermination empowerment and freedom of choice. The Allied Health Coordinator will assume a key role in coordinating and managing SWAMS clients considered for allied health services in Mandurah. The role will assist in developing overseeing and delivering services that promote effective and evidencebased prevention and management of clients who access SWAMS allied health services. The role will support the Mandurahbased allied health staff to provide highquality and effective patient care as well as to assist in monitoring KPIs and identifying gaps in the skills and knowledge of the team.

Role Responsibilities:

This role includes but is not limited to:

  • Providing support to the Mandurahbased SWAMS allied health team
  • Working collaboratively with SWAMS multidisciplinary teams clinic TCP service providers and NDIS service providers in client referrals and assessment.
  • Maintaining an uptodate record of patients including assessment care plan and summary of outcomes in the patients medical record to ensure good communication with other health professionals.
  • Providing clinical service and support in area of expertise (Physiotherapy/OT/RN) as required.
  • Assisting with the collection of statistics and data on all client contacts.

Essential Criteria

  • A tertiary qualification in Registered Nursing Physiotherapy or Occupational Therapy with full registration with the Australian Health Professionals Association.
  • A minimum of 5 years clinical experience in clinical area of expertise delivering services to clients with complex care needs.
  • Experience in communitybased allied health services.
  • Ability to take direction determine priorities and manage workloads to meet agreed timelines and objectives.
  • Good interpersonal written and oral communication skills.
  • Demonstrated ability to work as a team member and independently with minimal supervision.

Desirable Criteria

  • Demonstrated leadership management skills and proven capacity to lead and manage a multidisciplinary health team.
  • Demonstrated ability to develop plan implement and evaluate clinical and allied health care services to clients in a culturally appropriate way.
  • Aboriginal and Torres Strait Islander people are encouraged to apply
