Job Title: Social Worker St Helens Older People Mental Health
Location: St Helens

An exciting new role became available for a Social Worker with older people mental health experience.
What will your responsibilities be
You will be responsible for managing a caseload of older adults with diverse care and support needs. This role involves collaborating with multidisciplinary colleagues and other agencies to develop personalized care and support plans. You will support service users and carers in creating plans that utilize community resources to achieve positive outcomes. Additionally you must have the ability to travel across a wide geographical area to complete work for the Older Peoples Mental Health team.

Qualifications and Experiences:
You must have Social Work England registration be eligible to work in the UK and hold a full UK driving licence. Postqualified experience and a degree level or equivalent qualification in Social Work are also required.
