PRN RN Triage

Empath Health is seeking a PRN RN Triage to join our team.

Weekday night shift 12a8:30a and weekend hours 8am8:30pm. All counties within service area.

Uses the nursing process and care management strategies to coordinate care and provide nursing services for hospice patients and their families in the home long term care facility or assisted living facility.

Primary tasks duties and responsibilities:

1. Visits patients and provides clinical physical educational and emotional support afterhours as directed by the Triage team.
2. Under the direction of a licensed physician performs patient assessment manages discomfort and provides symptom relief using specialized nursing skills related to palliative and endoflife care.



Educational/Professional:

1. Current Florida Licensure as RN. BSN or MSN degree preferred.
2. Minimum one year recent acute care experience in a Hospital or Home Health environment
