Job Description

The CTO Coordinator works closely with  clients and families hospitals community agencies physicians and other mental health professionals to support the development implementation maintenance and monitoring of the community treatment orders in the catchment area of the central west region and/or Ontario Health Team (OHT). The CTO Coordinator has specialized knowledge on the Mental Health Act the substitute decision makers community treatment orders and consent and capacity in order to perform the position. The CTO Coordinator works with both internal and external partners for the delivery of the program and ensuring the patient is provided with as seamless service as possible.

Accountabilities:

  • Provides education to clients and all stakeholders including ACT teams community agencies and functions as a resource for CTO case managers and community case managers
  • Provides initial assessments of the client and development of individual CTO plan including liaising with community agencies to facilitate the implementation of the community treatment plan and attending family meetings on the inpatient unit while the plan is being developed
  • Provides psychoeducation to client and family throughout the duration of the community treatment order
  • Monitors the CTO clients progress on an ongoing basis and acts as a resource to family and community agencies on the plan
  • Completes referrals facilitates transfers and liaises with other coordinators and health professionals in other regions to ensure continuity of care and psychiatric followup for the client
  • Works closely with patient psychiatric advocacy office to ensure right advice is completed as per Mental Health Act
  • Submits forms for CCB hearings prepares templates and acts a resource for the CTO team in regards to CCB; attends hearings as required
  • Facilitates ongoing CTO renewals including meeting with clients and family and consulting with physician to evaluate client progress in order to assess whether ongoing CTO is required
  • Attends physician appointment with client and family to facilitate the process as per Mental Health Act
  • Attends relevant meetings for updates on Mental Health Act as well as CTO research and legislation and best practices
  • Ensures all named in the plan receive a copy of the plan and notifies the ministry of all CTOs completed via CTO information sheet
  • Works with clients physician case manager substitute decision maker Med clinic and police to facilitate a Form 47 as required

Qualifications :

  • Regulated health professional with a Bachelors degree in nursing social work occupational therapy or another clinical discipline
  • Minimum of five years demonstrated experience working with those with a serious and persistent mental illness and/or a concurrent disorder
  • A combination of experience and educational qualifications will be considered
  • Experience working in the mental health field in a hospital and or community setting
  • Current comprehensive knowledge of CTO legislation revised Mental Health Act Health Care Consent Act Substitute Decision Makers Act Regulated Health Professions Act and other appropriate legislation
  • Demonstrated ability to work successfully with community agencies and other stakeholders in developing creative partnerships to benefit individuals with serious mental illness
  • Excellent communication and problem solving abilities with ability to prioritize
  • Ability to take a leadership role in an intraprofessional team
  • Excellent assessment and intervention skills
  • Excellent interpersonal skills and strong written and verbal communication skills
  • Demonstrated ability to work within a diverse framework
  • Ability to work well both independently and within teams
  • Ability to travel between to all Osler sites

Additional Information :

DURATION: Approximately six 6 months or until the previous incumbent returns.

Hours: Currently days Monday to Friday 8 hour shifts in an onsite first role that frequently supports direct patient care and has dedicated hours when they are available to patients team members and/or other partners (subject to change in accordance with operational requirements)

Salary:
Minimum: $44.02 per hour
Maximum: $55.03 per hour
