Job Description

We are closely working with Achieve for Children for the appointment of an Childrens Community Nurse on a 3month contract likely to be extended at clients discretion. Please apply with your CV for immediate consideration.

Rate of Pay: 33.50 42.03 per Hour

Summary:

We are seeking an enthusiastic and proactive Childrens Community Nurse to join our dedicated Community Nursing Team. This role involves working autonomously across the borough to provide holistic child and familycentered care for children and young people aged 219 years with Special Educational Needs and Disabilities (SEND) Profound and Multiple Learning Disabilities (PMLD) and complex health needs. The ideal candidate will demonstrate strong clinical skills compassion and commitment to improving the health and wellbeing of children and their families.

Responsibilities:

  • Conduct comprehensive assessments to identify health needs and develop individualized care plans.
  • Plan deliver and evaluate specialist nursing care to ensure safe and effective support.
  • Provide clinical interventions such as gastrostomy management wound care suctioning and other specialist care procedures.
  • Deliver specialist training to families and school support staff on managing complex healthcare needs.
  • Liaise with multidisciplinary teams to ensure continuity of care and attend professional meetings including child protection case conferences.


Qualifications & Experience:

  • Registered Childrens Nurse (RNC) or equivalent qualification with valid NMC registration.
  • Experience working with children and young people with complex medical needs SEND or PMLD in a community or clinical setting.
  • Knowledge and experience in delivering specialist nursing care (e.g. gastrostomy care tracheostomy care wound management).
  • Strong assessment planning and evaluation skills to ensure highquality care delivery.
  • Excellent communication and interpersonal skills with the ability to work effectively with children families and multidisciplinary teams.
  • Ability to work autonomously and manage a diverse caseload across different settings.
  • Understanding of child protection procedures and safeguarding policies.
  • Experience in providing training and support to parents caregivers and education staff.
  • A valid UK driving license and access to a vehicle for community visits

  • You should be available to work immediately or at a short notice.
  • You should have right to work in U.K
  • This role requires an enhanced DBS
