Job Description
Position Summary
Provide personalized care and support to clients in the comfort of their homes ensuring a highquality compassionate service that meets the individual needs of each client. Work closely with the client and their family to promote independence safety and overall wellbeing.
Key Responsibilities
- Personalized Care: Assist clients with daily activities such as bathing dressing grooming and personal hygiene while encouraging their independence.
- Nutritional Support: Plan and prepare healthy nutritious meals tailored to the clients preferences and dietary needs. Assist with feeding if necessary.
- Household Support: Light housekeeping duties including laundry cleaning and organizing to maintain a safe comfortable and welcoming home environment.
- Companionship: Provide meaningful social interaction engaging conversation and emotional support to improve the clients overall quality of life.
- Health Monitoring: Observe and report any changes in the clients condition alerting the supervisor or healthcare professionals as necessary.
- Documentation: Maintain accurate records of services provided including dates and times ensuring clear communication with the healthcare team.
- Specialized Care: Under the guidance of a physician or healthcare professional assist with more specialized tasks as outlined by the Colorado Nurse Aide Practice Act.
- Other Duties: Assist with a range of tasks as needed ensuring the wellbeing and comfort of the client.

Qualifications :
Basic Qualifications & Experience
- High School Diploma or equivalent.
- 6 months experience working in an adult day healthcare or home care environment that serves senior adults.
- Colorado State Certified Nursing Assistant License in standing.
- Must possess a valid Colorado drivers license in good standing.
- Must possess current vehicle registration and proof of insurance coverage and reliable automobile.
- Must be able to read write and speak the English language.
- Must possess excellent customer service skills.
Working Conditions/Physical Requirements
- Position requires mediumheavy physical work. Heavy lifting pushing or pulling objects up to 100 pounds of force occasionally and/or up in excess of 45 pounds of force repeatedly to move objects.
- Stands and walks constantly during working hours with occasional sitting kneeling squatting and reaching above shoulder height and fine motor control with fingers. Also requires grasping.
- Exposure to blood borne pathogens body excretions and communicable diseases
- Is subject to frequent interruptions.
- Interacts with residents family members and visitors frequently.
