Job Summary
Apply the appropriate diagnostic and procedural codes to individual patient health information for data entry and retrieval analysis and claims processing. Patient record coding is completed in an accurate and timely manner according to department standards and in compliance with Coding Guidelines and other applicable coding rules and conventions.
Key Activities
ICD9CM codes are appropriately assigned to the OASIS tool (SOC ROC followup)
Appropriate codes are assigned and entered into the computer system prior to admission
Appropriate codes are assigned and entered at other OASIS time points as assigned.
Peer review is actively participated in to ensure coding accuracy.
Cross coverage is provided to support team to meet department objectives
Collaboration occurs with Utilization Management Specialist or Clinical Manager when code assignments are not straightforward or documentation in the record is inadequate ambiguous or unclear for coding.
Oasis workflow is monitored to maximize productivity
Accuracy and productivity standards are maintained in compliance with department standards
Standards of Ethical Coding and official coding guidelines are adhered to for accuracy
Coding changes and related publications are studied and information is kept current.
Additional department activities are completed as assigned and according to established objectives.

Education/Degree Licensure Certifications or Registration Requirements
CPC and HCSD home care coding
Previously demonstrated coding skills
Extensive recent coding experience of at least 2 years may be substituted for formal education and/or certification
Experience
Understanding of healthcare and applicable prospective payment systems and their relationship to the position required
Indepth understanding of health and medical record information required
Understanding of A & P medical terminology and record documentation required
Must be familiar with principles and practices of Health Information/Medical Records Sciences and understand the application to coding.
Must possess a knowledge and understanding of the ICD CM LTC and/or IRF coding contingent on the work setting.
Must have some familiarity with CPT coding and levels.
Computer proficiency required. Microsoft office applications preferred with ability to learn new software.
Highly developed communications and interpersonal skills working with diverse population.
Work independently be selfdirected and contribute as a member of a team.
Anticipates challenges and develops and implements strategies for addressing them.
High level of service delivery. Demonstrate initiative with ability to prioritize work meet deadlines and adapt to changing situations.
