Job Description
What you will do in this role:
The Restorative Nurse (RN or LPN) is responsible for coordinating restorative care for our center. You will assist residents in attaining or maintaining the highest level of physical functioning possible. You will work closely with our therapy team to help patients reach their clinical goals and prevent declines.

Qualifications :
What we need from you:
- Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in IL
- 2 years experience in a SNF setting as a Restorative Nurse is preferred
- The restorative/rehabilitation certification is ideal
- Electronic medical records experience is helpful (Point Click Care or PCC is ideal)

This center is managed by TRANSITIONAL CARE MANAGEMENT. We currently provide comprehensive management services to several inpatient healthcare centers throughout Northern IL & the Chicagoland area. These settings include Longterm Care Skilled Nursing (SNFs) Shortterm Care Rehabilitation Intermediate Care (ICFs) and Behavioral Health Service (SMHRFs) centers.
