POSITION SUMMARY:
The ambulatory clinical pharmacy specialist will be responsible for providing clinical pharmacotherapy services to patients under the care of the outpatient renal clinic. The specialist will work closely within multidisciplinary renal and pharmacy teams in the evaluation development and implementation of various clinical protocols chronic disease state management under collaborative practice agreement provide extensive patient counseling and provide leadership/mentorship to clinical pharmacists.
The renal pharmacy specialist will be heavily involved with direct patient care in the ambulatory setting transitional of care management (TCM) and postdischarge work.

ESSENTIAL RESPONSIBILITIES / DUTIES:
Evaluation and management of chronic conditions through direct patient care working under a collaborative practice agreement within the outpatient renal clinic. This includes but is not limited to Hypertension Chronic Kidney Disease Anemia and occasional coverage for the ambulatory pharmacist renal transplant service.
Teaching and precepting APPE PharmD students PGY1 and PGY2 practice residents
Collaboration with interdisciplinary healthcare professionals to improve and implement transitions of care (TCM) programs.
Development and assistance with various specialty drug management programs (ex: SGLT2i) Involvement with various quality improvement projects renal and pharmacy department initiatives and provider education
Actively participate in ambulatory formulary review process and various institutional committees
JOB REQUIREMENTS
EDUCATION:
Requires at a minimum a Bachelors Degree in Pharmacy and at least five years of pertinent work experience or a Bachelors Degree in Pharmacy and postgraduate training with at least three years of pertinent work experience or a Doctorate Degree in Pharmacy from an accredited college of pharmacy with postgraduate training or at least two years of relevant work experience.

CERTIFICATES LICENSES REGISTRATIONS REQUIRED:
Work requires licensure to practice pharmacy in the state of Massachusetts by the hire date.
Work requires substantial interpersonal skills towards our customers (particularly patients physicians and nurses) and fellow employees of the hospital.
KNOWLEDGE AND SKILLS:
Deep knowledge of medications their effects drug interactions medical procedures and therapies diagnosis and disease states required.
Effective interpersonal skills to interact appropriately with hospital staff patients families/visitors physicians colleagues and others.
Ability to multitask prioritize essential tasks and meet deadlines
An organized work style and ability to keep well written documentation.
Excellent verbal and written communication skills. Additional language skills can be a plus.
Must possess selfmanagement skills and require little to no supervision. This person must be a selfstarter selfmotivating have a strong work ethic and must have the ability to identify issues or opportunities that require attention and change.
Must possess and exhibit process improvement skills and acumen.
Strong attention to detail and accuracy.
Experience with Microsoft programs (i.e. MS Word Excel Outlook) and web browsers
