Job Description

TEMPORARY UNTIL APPROX. SEPTEMBER 26 2026

This position is responsible for delivering comprehensive supports tailored to individual client needs to assist them with progress toward successful employment and/or education goals. Provides social support assists in developing life skills and promotes learning opportunities that prepare clients for the experience of searching for finding and maintaining employment and/or education. Communicates and collaborates with the multidisciplinary team and community providers to promote client engagement enrolment and success. Carries out administrative duties as assigned for the daytoday operation of the program.

RESPONSIBILITIES & DUTIES:

  • Builds therapeutic relationships developing support plans with clients accessing community resources providing advocacy offering supportive counselling and crisis intervention.
  • Collaborates with the multidisciplinary team to review client needs and prepare a support plan/ discharge plan with specific resources/recommendations.
  • Assists in building a toolbox of skills for the client to use while exploring the world of work and education.
  • Will pursue ongoing engagement with community resources to assist the client with maximizing education/employment opportunities.
  • Developing specific employment/ education plans including goals activities expected outcomes and time frames which produce expected results and progress towards achieving identified employment/education goals of the client. These plans also reflect researched best practices and desirable outcomes and ensuring mechanisms are in place to demonstrate progress.
  • Developing an implementation plan which includes input from the client family members and other staff to address goals identify for schooling and employment needs.
  • Assists in the development of life skills such as time management household maintenance budgeting selfcare social life balance etc. These skills are the first step in determining client readiness and preparation to acquire work and/or explore education goals.
  • Provides a continuum of employment/education supports aimed at searching out securing and retaining advantageous employment/ education for clients of the program including skill building and development effective communication coaching and follow up support.
  • Creating and delivering learning experiences for enhancement of technology skills necessary for possible employment/education opportunities.
  • Liaising with High Schools Colleges Universities Windsor Social Services and other employment/education assistance programs to set job search continuums and to assist clients with the navigation through these platforms.
  • Under the direction of the Manager participates in the development of Quality indicators. program reports and trend analysis including monitoring the success of the service and amending the service as required to meet client needs.

EDUCATION & SKILLS:

  • Knowledge and skill acquired through the completion of a four 4 year degree such as a Bachelor of Social Work (BSW) Psychology Nursing (BScN) or Social or Health Sciences from an accredited University.
  • Registered member in good standing of a professional college deemed relevant by the employer such as the College of Nurses (preferably with Canadian Certification in Psychiatric/Mental Health Nursing) or the Ontario College of Social Workers and Social Service Workers or the College of Psychotherapy and in good standing is considered an asset.
  • Two years recent and relevant experience in the judgement of the employer working with vulnerable populations particularly with those who have experienced first episode psychosis.
  • Experience liaising with students parents guardians school personnel and external service providers to determine and respond to the needs of the students. Experience counselling students and clients experiencing personal crisis when faced with educational and vocational crisis. Previous experience working within a school environment is considered an asset.
  • Comprehensive knowledge of mental illness and treatments; functions and principles of case management. Knowledge of community resources specifically community education departments/ employment services.

Hours of Work: ThirtyFive 35 hour work week. The agencys regular hours of operation are Monday to Friday 8:30 AM to 4:30 PM. Flexible hours are required to meet client and service needs.

Salary range: Grade 8 $35.01 $41.42 per hour

PerCollective Agreement

Applicable shift premiums are paid in accordance with the Collective Agreement Article 17.11.
