The South West Aboriginal Medical Service (SWAMS) is an Aboriginal Community Controlled Health Service based in Bunbury that provides a variety of health services to Aboriginal people in the South West of Western Australia. SWAMS delivers a wide range of primary health and community programs and has a strong growth strategy through partnership opportunities and future community development.
This Aged Care Coordinator is responsible for the coordination of the contractual relationship between SWAMS and the Client ensuring their needs are met and quality services are delivered with respectful cultural considerations. The Aged Care Coordinator follows mutually agreed Client Care plans Aged Care National Standards and SWAMS policies and procedures.
The scope of the Aged Care Coordinator SW includes the following focus areas:
- Safety and Risk
- Leadership
- Service Planning and Quality
- Client Management
- Information Management
Key Duties included but not limited to:
- Coordinate services and activities that are culturally sensitive based on the clients individual assessed needs residing in their own homes.
- Manage and maintain the process relating to Client Care Plans to reflect required changes within the scope of assessed needs whilst helping clients to get the best possible services that suit their needs.
- Collect and collate client and service data review work practices and make recommendations for continuous improvement which result in improved service responsiveness to the Aboriginal community.
- Conduct or participate in research activities within the services as required.
- Coordinate performance appraisals and be responsible for contributing to professional development.
- Coordinate and/or support event preparation including all documentation accurately and in accordance with SWAMS policies procedures and processes.
- Identify and collaborate with other external service providers to ensure effective service delivery and increase community networks.
Specific requirements of this position include but are not limited to;
Essential Criteria
- Minimum Certificate III Individual Support or similar or willingness to enroll into qualification by the end of the probation period 6 months) and at least 3 years experience working in an aged care setting coordinating services.
- Demonstrate respect and understanding of client issues including cultural appropriateness and antidiscrimination
- An understanding of the health and social issues impacting Aboriginal people.
- Demonstrated experience in coordinating activities and teams to achieve outcomes and high performance
- Strong written and interpersonal skills and the ability to communicate effectively and sensitively with Aboriginal and Torres Strait Islander persons.
- Excellent organisational and problemsolving skills.
- Experience in undertaking complex assessment and care coordination of client services including collating and interpreting statistics.
