Job Description
Role: Ward Clinical Pharmacist
Banding: Band 8a
Location: Westcliffonsea
Start Date: ASAP
Working Pattern: Monday to Friday 09:00 17:30pm
Rate: 34 39 per hour
We are seeking a highly skilled and experienced Wardbased Clinical Pharmacist to join our team in WestcliffonSea. The successful candidate will provide comprehensive clinical pharmacy services on the wards ensuring the safe and effective use of medications. This role is essential in maintaining high standards of patient care and supporting the multidisciplinary team in delivering excellent service.
Duties and Responsibilities:
In this role you will be responsible for providing clinical pharmacy services on the wards including medication reconciliation clinical screening and patient counselling. You will work closely with the healthcare team to optimize medication therapy manage discharge medications and ensure seamless transitions of care. Your role will also involve participating in ward rounds supporting clinical audits and contributing to quality improvement initiatives within the pharmacy department.
Requirements:
- Essential:
- Up to date GPHC registration.
- Relevant Pharmacy qualification.
- Experience in a clinical pharmacy setting.
- Desirable:
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
- Excellent organizational and timemanagement skills.
