JOB SUMMARY
The Certified Occupational Therapy Assistant performs services planned, delegated and supervised by the Occupational Participates in the rehabilitative plan of care designed by the Occupational Therapist and physician, and monitored and evaluated by the Occupational
QUALIFICATIONS
Graduate from an accredited Certified Occupational Therapy Assistants program with an Associate s Degree approved by the American Occupational Therapy Licensed or certified by the state in which the Certified Occupational Therapy Assistant resides or practices according to state requirements, and has received certification from the National Board for Certification in Occupational Therapy, (NBCOT).
PHYSICAL DEMANDS
Requires moderately heavy physical exertion on a regular and reoccurring basis such as extensive driving, assisting patients in transfer activities (wheelchair to bed, to tub, to commodes) and providing substantial support to individuals in ambulation Regularly required to use hands and Frequently required to stoop, kneel, crouch, or Occasionally lift and/or move over 100 Specific vision abilities include close, peripheral, depth perception and ability to adjust

ESSENTIAL FUNCTIONS
Participates in the ongoing evaluation of the patient s functional status (muscle function, endurance, visual coordination, written and verbal communication skills, self-care ability, work capacity, ) as delegated by the Occupational Observes, records and reports to the Occupational Therapist and Program Supervisor responses treatment and any changes in the patient s
Treats the patient in accordance with the care plan after the Occupational Therapist performs the initial evaluation to establish treatment goals, frequency, duration and specific modalities to be used in accordance with the physician s orders with the Occupational Following treatment program and goals for improved patient function as established by the Occupational
Reviews weekly patient s Contacts the Occupational Therapist directly for patient reports and care as needed, or required by State/Federal
Submits appropriate written reports of the patient s progress at the designated intervals to the physician and/or the Occupational All notes written by the Assistant will be co-signed (if state requires) by the supervising Occupational
Instructs patients regarding occupational therapy procedures designed by the supervising Occupational
Adheres to all applicable federal, state, local and company-maintained standards of care and ethics and policies concerned with the practice of occupational therapy and among other things, personnel
Participates in staff development activities and in in-services education
Meets productivity and utilization expectations as established by the Company and the
Completes clinical documentation to include functional goal-setting, progress and therapeutic approach timely and
Completes administrative/billing documentation timely and
