COTA PRN Skilled Nursing

JOB SUMMARY
The Certified Occupational Therapy Assistant performs services planned, delegated and supervised by the Occupational Therapist. Participates in the rehabilitative plan of care designed by the Occupational Therapist and physician, and monitored and evaluated by the Occupational Therapist.

QUALIFICATIONS
Graduate from an accredited Certified Occupational Therapy Assistants program with an Associate s Degree approved by the American Occupational Therapy Association. Licensed or certified by the state in which the Certified Occupational Therapy Assistant resides or practices according to state requirements, and has received certification from the National Board for Certification in Occupational Therapy, Inc. (NBCOT).

PHYSICAL DEMANDS
Requires moderately heavy physical exertion on a regular and reoccurring basis such as extensive driving, assisting patients in transfer activities (wheelchair to bed, to tub, to commodes) and providing substantial support to individuals in ambulation therapy. Regularly required to use hands and arms. Frequently required to stoop, kneel, crouch, or crawl. Occasionally lift and/or move over 100 pounds. Specific vision abilities include close, peripheral, depth perception and ability to adjust focus.

WORKING CONDITIONS
General office, health care setting - exposure to infectious disease.

ESSENTIAL FUNCTIONS
1. Participates in the ongoing evaluation of the patient s functional status (muscle function, endurance, visual coordination, written and verbal communication skills, self-care ability, work capacity, etc.) as delegated by the Occupational Therapist. Observes, records and reports to the Occupational Therapist and Program Supervisor responses treatment and any changes in the patient s condition.

2. Treats the patient in accordance with the care plan after the Occupational Therapist performs the initial evaluation to establish treatment goals, frequency, duration and specific modalities to be used in accordance with the physician s orders with the Occupational Therapist. Following treatment program and goals for improved patient function as established by the Occupational Therapist.

3. Reviews weekly patient s programs. Contacts the Occupational Therapist directly for patient reports and care as needed, or required by State/Federal guidelines.

4. Submits appropriate written reports of the patient s progress at the designated intervals to the physician and/or the Occupational Therapist. All notes written by the Assistant will be co-signed (if state requires) by the supervising Occupational Therapist.

5. Instructs patients regarding occupational therapy procedures designed by the supervising Occupational Therapist.

6. Adheres to all applicable federal, state, local and company-maintained standards of care and ethics and policies concerned with the practice of occupational therapy and among other things, personnel qualifications.

7. Participates in staff development activities and in in-services education programs.

8. Meets productivity and utilization expectations as established by the Company and the facility.

9. Completes clinical documentation to include functional goal-setting, progress and therapeutic approach timely and accurately.

10. Completes administrative/billing documentation timely and accurately.

11. Covers more than one facility as required by the need of the Company.

Observes, records and reports to the physician the patient s reaction to the therapy program or any changes in the patient s condition. 5. Advises, consults and instructs patients in the use of prosthetic, orthotic and assistive devices. Instructs, teaches and supervises other health care team personnel in the case of patients requiring occupational therapy. 6. May supervise Certified Occupational Therapy Assistant(s) and co-sign Certified Occupational Therapy Assistant s progress notes. 7. Adheres to all applicable federal, state, local and company maintained standards of care and ethics and policies concerned with the practice of occupational therapy and among other things, personnel qualifications. 8. Participates in staff development activities. Participates in in-services education programs. 9. Demonstrates evidence of continuing education conducted for the in-service sessions. 10. Meets productivity and utilization expectations as established by the Company and the facility. 11. Completes timely clinical documentation to include functional goal-setting, progress and therapeutic approach. 12. Completes clinical documentation timely and accurately. 13.
