Responsibilities
Responsible for providing psychosocial-emotional support and services to patients and families. Assists patient and family in understanding coping and adapting to the personal emotional and bio-psychosocial issues associated to the patients illness. Provides comprehensive assessments and referrals crisis intervention and care management. Collaborates with multi-disciplinary team and other internal and external personnel to promote an awareness of environmental and psychosocial needs and to develop and implement treatment plans that support patient-centered plan of care for both individual patients and the medical community. This position promotes individual professional growth and development by serving as a preceptor and providing individual and group mentorship to unlicensed Social Workers and interns.
Qualifications
Education
Masters Degree in Social Work (MSW) required
Experience
2 years of experience in psychosocial and therapeutic assessments and interventions required
Licenses and Certifications
BLS - Current Basic Life Support (BLS) for Healthcare Providers by the American Heart Association (AHA) required
LCSW - State of California Licensed Clinical Social Worker required
