Promotes early intervention and ensures coordinated continuous care for children and families in underserved communities. Through the use of the Collaborative Care Model (CoCM) and telehealth the coordinator facilitates integrated treatment planning strengthens school-clinic partnerships engages families and connects them to essential resources. The role also supports outcome tracking quality improvement initiatives and efforts to address social determinants of health.

License/Certifications: Current Texas licensure as an LPC-A LMFT-A LMSW LPC LMFT or LCSW; Qualified Mental Health Professional (QMHP) OR eligible for QMHP certification (based on minimum and defined education above); Valid Texas Drivers License
