Job Summary

Promotes early intervention and ensures coordinated continuous care for children and families in underserved communities. Through the use of the Collaborative Care Model (CoCM) and telehealth the coordinator facilitates integrated treatment planning strengthens school-clinic partnerships engages families and connects them to essential resources. The role also supports outcome tracking quality improvement initiatives and efforts to address social determinants of health.

Minimum Education: Bachelors Degree

Defined Education: Health and Humans Services field of Applied Behavior Analysis - Child Development Counseling - Early Childhood Education - Early Childhood Intervention - Education Psychology Gerontology - Human Growth and Development Medicine Nursing - Physician Assistant Psychology - Registered Nurse - Rehabilitation & Rehab Studies - Social Work Sociology - or Special Education

Preferences: Masters Degree; Bilingual-Spanish

Substitutions: Bachelors Degree with completion of college level coursework equivalent to requirements for QMHP credential

Years Experience: One (1) year

Defined Experience: Health and Human Services mental health or behavioral health-related field

License/Certifications: Current Texas licensure as an LPC-A LMFT-A LMSW LPC LMFT or LCSW; Qualified Mental Health Professional (QMHP) OR eligible for QMHP certification (based on minimum and defined education above); Valid Texas Drivers License
