Job Summary

On 1 January 2026 Alfred Health will come together with Kooweerup Regional Health Service Peninsula Health Bass Coast Health and Gippsland Southern Health Service to create Bayside Health a newly merged health service with more than 22000 employees caring for a community of over 1.2 million Victorians. The new health service will provide people with the best possible care with services that support them throughout their lives. The merger will also strengthen our workforce providing new training education and career opportunities. Subject to final approval on 1 January 2026 all employees from the five health services will transfer to Bayside Health.

The Department

The Commonwealth Department of Health and Aged Care has established a new Single Assessment System (SAS) to improve access to aged care services. As of January 2025 this system has replaced the Aged Care Assessment Service (ACAS) and Regional Assessment Service (RAS).

Alfred Health Aged Care SAS provides comprehensive and home support assessments via the Victorian Dept of Health agreement. Comprehensive assessments are usually completed face to face in the hospital or community setting to determine a clients eligibility for aged care types under the Act. Home support assessments are completed in a community setting for clients identified as having entry level needs that can be accommodated by Commonwealth Home Support Services.

The geographical catchment for community assessments incorporates the Southern Metro Aged Care Planning Region.

The Role

This role will require you to travel to complete face- to-face assessments as well as telehealth and phone assessments as required.

You will need to be adaptable to assessment demand and may be required to complete home support assessments. You will be involved in training for the new Aged Care Act and Support at Home Program which commenced 1st November 2025.

Experience and Qualifications Required

This dynamic and supportive interdisciplinary team is seeking applications from suitably qualified health professionals (Nurse Occupational Therapist Physiotherapist Social Worker); who must have advanced assessment skills community and aged care experience.

Allied Health

  • Degree or equivalent in a relevant Allied Health discipline
  • AHPRA (Australian Health Practitioners Regulation Agency) registered or eligible for professional membership if a self-regulated Allied Health profession

Nursing

  • Degree in Bachelor of Nursing or equivalent
  • AHPRA registered

Highly desirable

  • Recent SAS ACAS or RAS experience and current compliance of all My Aged Care (MAC) learning modules.
