Director of Case Management
Compensation: $100000 $112000 annually plus eligibility for quarterly bonus
Relocation Assistance

An excellent opportunity is available for an experienced leader to step into the role of Director of Case Management at a 60-bed acute care hospital with an average daily census of 4648 patients in 2024.
Position Overview
The Director of Case Management oversees a dynamic team of 7 direct reports including 4 full-time case managers (2 RNs and 2 social workers) 1 part-time case manager assistant and 2 per diem case managers. This leader is responsible for advancing patient-centered case management utilization review and discharge planning practices while ensuring compliance with state and federal regulations.
This role requires a leader who is results-driven collaborative and able to foster effective communication across clinical teams to ensure optimal patient outcomes and efficient transitions of care.
Key Responsibilities
- Direct and manage all case management operations including utilization review discharge planning and care coordination.
- Mentor and develop a diverse team of case managers and support staff.
- Partner with clinical and administrative leadership to improve patient flow outcomes and resource utilization.
- Monitor performance metrics and implement process improvements to meet quality and regulatory standards.
Candidate Profile
- RN preferred; must hold a current state license (RN or Social Worker).
- If nursing: Bachelors degree in Nursing (BSN) required.
- If Social Worker: Masters degree in Social Work required.
- Certification required: CCM or ACM to be obtained within 12 months of hire.
- Experience: Minimum of 3 years in hospital-based case management including utilization review and discharge planning.
- Proven leadership skills with ability to manage teams in an acute care environment.
