Arise / EFR is looking for a Housing and Care Coordinator
The Housing and Care Coordinator provides information about the ESSHI supportive housing program at Sennett Meadows to the community determines program eligibility for referred individuals and coordinates all start-up activities to ensure successful establishment of residence. Responsible for addressing and resolving all housing related issues and supporting the goals of approximately nine program participants. Obtain needed community-based services and provide one-on-one support to maintain or increase each participants ability to live safely and successfully at Sennett Meadows. This position conducts in-depth case management with each participant and is responsible for addressing all identified independent living needs. The Housing and Care Coordinator is responsible for ensuring all physical mental and behavioral needs are assessed and addressed.

Qualifications: - Bachelors degree preferred; or equivalent combination of education and related experience working in the disability advocacy field and/or with persons with disabilities
- Knowledge of community services housing regulations and disability support programs
- Excellent advocacy skills and ability to form collaborative relationships
- Must be able to skillfully address sensitive situations and problem-solve while maintaining the integrity of the program
- Clean driver license and reliable insured inspected vehicle
- Computer Proficiency

Essential Functions: - Work closely with referral sources to identify potential program participants
- Assess program eligibility and suitability with each person referred and complete enrollment process
- Work with participants to identify and secure essential household items furnishings and initial food supply; help set up apartment
- Conduct a comprehensive needs assessment for each participant to identify support services that help maintain independence
- Educate participants and family members on available services and resources
- Provide general case management to link participants to needed services such as health and wellness preventative personal assistance transportation food legal financial etc.
Work location: Cayuga County (Auburn & Sennett) Hours and days: Monday-Friday 8:00am -4:00pm Compensation: $46000 -$50000 Annually Non -Exempt; 40 hours per week includes one-hour paid lunch |
