Overview
We are seeking a compassionate and skilled Home Care Physical Therapist to join our care team in Fort Dodge this full-time role you will provide high-quality rehabilitative and preventative physical therapy services to patients in their homes. This position offers an opportunity to make a meaningful impact on patients quality of life through personalized care education and support.

Key Responsibilities

Conduct home visits for patients requiring home health physical therapy services
Perform initial and comprehensive patient assessments
Evaluate and document each patients functional status and rehabilitation needs
Provide treatments to relieve pain and restore movement and function
Develop implement and update individualized plans of care
Initiate and perform diagnostic preventive and rehabilitative procedures
Ensure safe and effective delivery of all therapy services
Promote patient independence through education and exercise programs
Maintain accurate and timely clinical documentation
Communicate with physicians and interdisciplinary team members regarding patient progress
Supervise home health aides and physical therapy assistants as needed

Schedule and Shift Details

Monday through Friday 8:00 a.m. to 4:30 p.m.

Qualifications

Required

Bachelors degree or higher in Physical Therapy
Active Physical Therapist license in the state of Iowa
Proficiency with Microsoft Office and EMR documentation
Strong communication time management and organizational skills
