SoCO Services Program Manager

Position Description:

Enhances the quality of life for people receiving long term care services to include individuals with intellectual or developmental disabilities receiving residential day employment and supported living services by providing direct support to individuals and families as well as teaching & training Direct Support Professionals so they can provide mentoring and support to the individuals in services. The SoCO Services Program Manager will be responsible for the expansion of PPCH services into the Southern Colorado region. This position will work 30 hours per week and may become full-time over time and as business needs dictate. This position is eligible for company-sponsored benefits.

Essential Duties:

1. Support the Senior Executive Support Director in ensuring that services and supports are provided in a manner that respects and acknowledges individuals self-expression self-worth self-reliance and decision making.
   • Encourages a culture of collaboration with individuals receiving services their loved ones and other support teams.
   • Ensures that staff have the training resources and work environment to support individuals receiving services in pursuing their goals activities of interest and social interactions.
   • Provides effective and person-centered services through implementation of Service Plans Individual Service and Support Plans and recommendations of team members.
   • Works continuously to improve services and supports using a person-centered approach.

1. Supports a culture where individuals receiving services are healthy safe and valued by others.
   • Models and teaches respect for others including individuals in services staff guardians and loved ones and team members.
   • Ensures the safety well-being and satisfaction of individuals receiving services by providing training and mentoring to staff.
   • Arranges for staff coverage of essential needs as outlined in Service Plan.
   • Assists people in Independent Living (IL) and Supported Living Services (SLS) with their medical needs by helping with appointment scheduling transportation to appointments communication with medical providers and medication management when needed.
   • Assists people in residential services by ensuring quality assurance of all Host Homes on caseload (including meeting standards of agency and governing bodies.) This includes ensuring that documentation remains current that Host Home Providers remain up to date on programmatic documentation and completion of quarterly home assessments as needed and according to individual plans.
   • Assists people in Employment services by providing on-site training instruction supervision and advocacy for individuals with developmental disabilities at community job sites.
   • Supports people receiving IL services in securing and maintaining safe and comfortable housing. This includes support with furnishings supplies and interactions with property managers and landlords.
   • Assists people in day services to participate in their community according to their interests desires and ISSP
     1. Establishes and follows a calendar of activities
   • May provide support with personal care homemaking mentorship grocery shopping budgeting and household tasks.
   • Encourages and supports independent decision-making and self-advocacy.
   • Works continuously to improve services and supports.
   • Assesses community locations activities and PPCH vehicles on a regular basis and reports concerns to supervisor.

1. Expands PPCH supports and services into the Southern Colorado area with support from Senior Executive Support Director.
   • Attends service inquiry meetings
   • Provides accurate service information and descriptions
   • Monitors budget which informs hiring decisions and service capability
   • Hires staff for various services in region
   • Communicates with Director Of for each service being provided to ensure consistent supports

1. Complies with PPCH policies and the rules and regulations of State and Federal oversight agencies.
   • Completes required trainings including annual refreshers according to established timelines.
   • Accurately updates and manages the Personal Needs Funds and individual information of people receiving services using designated software and procedures.
   • Maintains knowledge and understanding of PPCH and other regulating agencies policies procedures rules regulations and guidelines.
   • Completes programmatic documentation required by PPCH or any regulatory agencies accurately and in a timely manner. Audits documentation for those receiving services on a weekly basis.
   • Inventories the belongings of people receiving services upon move-in and annually thereafter.

1. Ensures that the self-expression self-worth self-reliance independence and decision-making of individuals receiving services from PPCH is supported and respected.
   • Assists with the onboarding and training of new contracted providers including customer-specific training (CST).
   • Responsible for the daily operation of a caseload for people who receive services across PPCH programs for the first 30 90 days as needed.
   • Provides support and services to new admissions on their caseload including:
   • Assisting with onboarding of new people receiving services and host home providers
   • Documentation training for new providers
   • Creation of the required PPCH Client Folder including
     • Consent packets
     • HRC if needed
     • PARs and Service Plans
     • IDT attendance
     • CST
     • Host home visits and audits
   • Ensures that training resources and work environment are provided to support individuals receiving services in pursuing their goals activities of interest and social interactions
   • Attends team meetings and ensures implementation of the person-centered service plan and applicable rights modifications for people receiving services
   • Works closely with the directors of PPCH programs to ensure a smooth transition into services
   • Evaluates and assesses the programs strengths and weaknesses to improve services and supports using a person-centered approach

Other Duties:

1. Attends PPCH team meetings and interdisciplinary team meetings as needed or requested.
2. Works in the capacity of direct care professional or other staff as needed.
3. Maintains and tracks supply / inventory.
4. Other duties as assigned.

Qualifications:

Education and Training:

• High school diploma or equivalent required.
• Bachelors degree from an accredited college or university strongly preferred.
• Required training will be provided and must be successfully completed.

Experience:

• Minimum of one year experience working with individuals with Long Term Care waiver support needs required.
• Minimum of one year experience working in the Colorado Long Term Care System required.
• Minimum of one year using Microsoft Office or other software required.
• Supervisory experience strongly preferred.
• Program management experience strongly preferred.
• Experience building and managing a budget strongly preferred.

• Ability and willingness to complete required trainings and learn PPCH operations and systems.
• Valid Colorado drivers license and ability to meet PPCHs driving requirements.
• This position may require occasional use of a personal vehicle with liability insurance that meets PPCH standards. When possible staff are strongly encouraged to use PPCH owned vehicles.
• Possess a strong knowledge of Social Services and out-of-home placements with a particular emphasis on adults with Intellectual and/or Developmental disabilities.

Physical Requirements and Working Conditions:

• Work is often performed in private homes or in the community. Work is occasionally performed in a typical office setting.
• Frequent lifting and carrying of approximately 15 lbs.
• This position involves physically assisting others including pushing/pulling individuals using wheelchairs transferring individuals with mobility needs and assistance with daily living skills.
• Lifting moving and transferring up to 75 pounds.
• Work is primarily conducted in private homes office settings and the community.
• Driving throughout the Pueblo / Colorado Springs area required on a regular basis.
