Job Summary

3 months contract with a Local Authority
Job Summary:
An interim opportunity has arisen for an experienced Personalised Commissioning Manager to join an Integrated Commissioning Team on a project basis.
The successful candidate will lead on personalised commissioning arrangements for adults with learning disabilities autism and mental health needs.
The role involves overseeing complex individual placements driving service transformation and working collaboratively across health and social care systems to improve outcomes and reduce reliance on inpatient care.
This post sits within an integrated team jointly operated between local authority and NHS partners focusing on individualised care coordination and strategic commissioning for vulnerable adults.

Key Duties/Accountabilities (Sample):
Lead commissioning and oversight of Care (Education) and Treatment Reviews (C(E)TRs) and Dynamic Support Register (DSR) processes.
Manage a small caseload of individuals with complex needs coordinating funding care planning and provider engagement.
Collaborate with clinicians local authority teams NHS colleagues and providers to support appropriate placements and reduce hospital admissions.
Monitor project delivery and provide regular updates on outcomes and financial management.
Contribute to service development quality assurance and market shaping across supported living employment and housing pathways.
Chair multi-agency and stakeholder meetings and represent commissioning at strategic forums.
Support equality diversity and inclusion in commissioning practice and service delivery.
Facilitate workforce development and knowledge sharing across care teams on commissioning tools and systems (e.g. DSR C(E)TRs).
Coordinate internal reporting and maintain accurate financial records for individual placements.
Promote adherence to Section 117 funding safeguarding and legal responsibilities in all placements and commissioning activity.

Skills/Experience:
Extensive experience in commissioning or providing services for adults with learning disabilities autism and/or mental health needs.
Strong understanding of C(E)TRs DSR and Section 117 aftercare funding processes.
Proven track record of managing complex cases and facilitating multi-agency care coordination.
Deep knowledge of NHS and local authority commissioning structures policies and governance.
Ability to analyse and interpret financial and performance data to inform decision-making.
High-level stakeholder engagement and communication skills with ability to write and present reports to senior stakeholders.
Proficiency in project management service redesign and performance monitoring.
Confident IT skills including Microsoft Office commissioning databases and digital collaboration tools.
Experience of working with elected members senior clinicians social workers and third-sector providers.
Understanding of key national programmes such as the Learning Disability and Autism Programme Annual Health Checks and LeDeR reviews.

Additional Information:
Working Pattern: Hybrid (some office/placement location attendance required).
Hours: 37.5 per week.
